Identity stays out of manager views

The response is associated with an employee on the server, but manager-facing survey outputs do not include employee identity.

No IP or fingerprint fields on responses

The survey response record does not have fields for IP addresses or browser fingerprints.

Scores have a hard floor

Quantitative reporting has a configurable threshold with a hard floor of five responses.

Comments use a separate threshold

The open-text threshold defaults to seven, cannot be lower than the score threshold, and can be higher for sensitive segments.

Use this six-question structure as a starting point for a short anonymous feedback form. Give the first question a 0 to 10 scale. Leave the rest open so people can explain what is happening in their own words.
